Step 1: Referral Form
By providing details in the referral form, we can set up a file for you in preparation for our service.

My Health and Mental Wellbeing services include NDIS Support Coordination in the Central Coast and Lake Macquarie area.
If you care for an NDIS participant or are a participant yourself, our experienced Support Coordination can help you make the most of your NDIS plan. Our company seeks to link you with support and providers that are a good fit for you. Where we use our own counselling service, we aim to use strength-based and person-centred approaches. We must advise you of a potential conflict of interest. Should there be an issue, we will seek to connect you with Mental Health support from your local area.
Our Process
Step 2: Meet and Greet
This session is important for us to get to know each other. We need to build a level of trust to begin building our therapeutic alliance. We need to demonstrate that we have your best interests at heart.
We will discuss your goals and objectives with you to ensure we are on the same page in understanding your needs.
Step 3: Onboarding
You can conveniently access our coordination services online via web, phone, SMS, or email, or we can visit you in the comfort of your own home. For your support needs, contact us at 0429 510 612 or email us at damian@myhealthandmentalwellbeing.com
1. Send an email with the intake form attached. Access to an Online document for easy completion and signing (we can talk you through this to make life easier)
2. Send an email with the confidentiality agreement attached. (Signature required)
3. Send the finalisation of our service agreement. Signature required. Must be copied to ‘you’ ‘NDIS’ or ‘Plan Manager.’
Step 4: Support Begins
After our meet and greet session, we will have a good idea of the support required and the times you require it.
We will contact you initially to ensure we meet your expectations in the second week of services. As our systems are fully automated, we can arrange a fill-in if support becomes unavailable to meet your support requirements. If your needs change for some reason you don’t require support on a day, please let us know 24 hours prior to avoid cancellation costs.
